In 1 of 1 (Pt. #2) clinical records with Aide services, the Agency failed to ensure that the Aide performed duties as assigned.

In 1 of 1 (Pt. #2) clinical records where in the Pt. received Aide services, the Agency failed to ensure the RN conducted Aide supervision every 14 days.

In 1 of 2 (Pt. #6) closed clinical records reviewed, the Agency failed to ensure the Physician or health care facility where patient was receiving care, was sent a copy of the Transfer summary and/or Discharge summary.

The Agency failed to ensure that clinical documents were complete.
