Methodology & disclaimer
Sources
- Agency list: CMS Provider Data Catalog, "Home Health Care Agencies" (dataset 6jpm-sxkc, file dated 2026-05-27).
- Findings: CMS QCOR "HHA 2567s" full extract (inspections 2018-01-02 to 2026-10-15, downloaded 2026-10-20).
- Last updated: 2026-10-20. Covers all 50 states, DC, Puerto Rico, Guam, the U.S. Virgin Islands and the Northern Mariana Islands.
Severity labels
- Serious = condition-level citation (the agency failed an entire Medicare Condition of Participation).
- Moderate = standard-level citation.
- Minor = element-level citation.
- Initial survey comments ("memo" rows) are not counted as deficiencies.
Plain-English summaries
Each summary is the inspector's own sentence stating what the agency failed to do, copied from the Form 2567 text with only introductory phrases (such as "Based on record review and interview,") removed. No facts are added or changed. If no such sentence can be extracted, the rule's title is shown instead. The full inspector text is available under every summary ("Show inspector's text").
Agencies no longer in CMS's list
Agencies that have inspection records but are not in CMS's current list are shown with a "no longer in CMS list" label, because past findings can matter when an operator reopens under a new name. Their address comes from the last inspection report.
Limits
Plans of correction are not in CMS's public extract. Agencies accredited by private organizations may be inspected by those bodies, whose reports are not public, so "no findings" does not mean "no problems." Records are matched by CMS Certification Number. Errors: report them and we will correct them.
