The Home Health Agency (HHA) failed to enter into the patients clinical records the following documentations: 1. Missed Skilled Nurse (SN) visits.

The Home Health Agency (HHA) failed to ensure patient's current medications (Medication Reconciliation) were complete, accurate and reconciled in accordance with the current plan of care for one of seven sampled patients (Patient 5).

The Home Health Agency (HHA) failed to indicate the frequency of skilled nurse (SN) visits in the plan of care of one of seven sampled patients (Patient 2).

The Home Health Agency (HHA) failed to notify the physicians about the skilled nurses missing visits for three of seven sampled patients (Patient 3, 1, and 6).

The Home Health Agency (HHA) failed to ensure skilled nursing visit were provided as indicated in their plan of care for three of seven sampled patients (Patient 3, 1, and 6).

The Home Health Agency (HHA) failed to ensure the Registered nurse (RN) made an on-site supervisory visit every month for skilled nursing (SN) for six of seven sampled patients (Patient 3, 2, 1, 6, 7, and 5).
