The Skilled Nurse (SN) failed to include all medications during the review of the patient's medications in 2 (# 3, #9) of 17 (#1-#17) clinical records reviewed.

The Agency failed to review and update the emergency preparedness plan on an annual basis.

The Skilled Nurse (SN) failed to establish an individualized Plan of Care (POC) with patient specific blood glucose parameters for 6 (#4, #6, #7, #10, #14, and #16) of 7 (#4, #6, #7, #10, #13, #14, and #16) Clinical Records reviewed with endocrine interventions.

2. During an interview on 11/15/18 at 2:15 PM, Agency Personnel #3 stated the Agency staff verbally informed the patients of changes or revisions in the POC and confirmed the Agency did not provide written information to the patients for updates or as the Plan of Care changed .

The Physical Therapist (PT) failed to complete the initial assessment on the Physician's ordered Start of Care (SOC) date in 1 (#9) of 2 (#9 and #11) Clinical Records reviewed with orders for PT Start of Care to Evaluate and Treat.

The Skilled Nurse (SN) failed to include all medications the Patient was currently using during a review of the patients medication regimen in 3 (#8, #13 and #17) of 17 (#1 - #17) Clinical Records reviewed.

The Skilled Nurse (SN) failed to follow accepted infection control standards 1 (#15) of 4 (#12,#13,#15 and #16) home visits with observations of skilled procedures performed.
