The agency failed to ensure that patients were informed about and consented to the frequency of proposed visits in advance of starting care in two (2) of five (5) clinical records reviewed in the survey sample (clinical record #'s 1 and 3).

The agency failed to provide a Notice of Medicare Non-Coverage (NOMNC) to Medicare patients in advance of terminating care for one (1) of one (1) closed/discharge clinical records reviewed in the survey sample for which a NOMNC was applicable.

The agency's professional staff failed to promptly alert the patient's physician and/or wound clinic to any changes that suggest a need to alter the patient's plan of care for one (1) of two (2) medical records in the survey sample that required wound care (Medical record #1).
