The agency failed to coordinate services and inform the physician and/or other personnel of changes in the patient's condition/needs and/or initiate plan of treatment revisions for two (2) of six (6) of active clinical records reviewed (CR1, CR2).

The agency failed to ensure Skilled Nursing services were received as per the physician orders as written on the plan of care for two (2) of nine (9) active clinical records reviewed (CR6, CR9).

The facility failed to document that a summary of the patient's progress was sent to the physician at least once every sixty (60) days per agency policy for two (2) of two (2) clinical records reviewed that contained recertification periods (CR5, CR9).

The agency failed to notify the physician of patient change in status and/or coordinate care delivery to meet the needs of the patient for one (1) of four (4) of closed clinical records reviewed (CR4).
