The home health agency (HHA) failed to ensure the pressure wounds (localized, pressure-related damage to the skin and/or underlying tissue usually over a bony prominence) for one of 7 sampled patients (Patient 5) was documented accurately.

The agency failed to ensure infection control practices were followed for one of seven sampled patients (Patient 7) when the agency staff did not perform hand hygiene during the home visit.

The Home Health Agency (HHA) failed to ensure the medication profile was reviewed for one of seven sampled patients (Patient 3).
