The agency had actual emergency’s that required the activation of their relevant emergency plan that exempted engagement in a community-based full-scale exercise in 2021, 2022, and 2023. The agency did not have sufficient documentation of a secondary exercise as required by 484.102(d)(2)(ii).

Adequate infection control prevention was not implemented during the provision of wound care by the RN.

The agency staff did not communicate delays in the start of care were noted for 11 out 17 reviewed placing the patients at risk for adverse or negative health care outcomes.

The agency staff failed to appropriately individualized care plans for each patient receiving home health services.

Infection control prevention was not implemented during the provision of patient care by the agency staff for 1 out of 7 patients observed.

The agency's staff did not provide 1 out of 1 newly admitted patient with the required patient rights information.

The agency staff failed to provide home health aide services to the patient.

Effective teaching was not provided by the clinical staff to patients and/or their caregivers for 6 out of 17 patient records reviewed.

Effective coordination of care with the patient's physician was not performed by the agency staff for 4 out of 17 patient records reviewed.

The clinical staff failed to properly document home health aide missed visits and failed to notify the registered nurse that the visits were not performed.
