The RN failed to follow appropriate infection control policies during observed wound care for 1 of 2 patients receiving a joint nursing visit [# 7]. The policy, Wound dressing application, home care [2/24/25], was provided by RN # 3 on 9/23/25 at approximately 2:25 pm.

Direct observation the agency staff failed to use proper wound care technique for 1 of 7 virtual home visits (#2), failed to use proper hand hygiene for 1 of 7 virtual home visits (#2) and failed to use proper venipuncture draping technique for 1 of 7 virtual home visits (#2).

Staff interview the agency failed to provide home health aide supervision for 1 of 4 home health aide records reviewed (#14).

The agency failed to obtain a PRN [as needed] order [from the physician] to assess the patient for a change in condition for for an additional visit for 1 of 20 patients (Patient #7).

The skilled nurse failed to assess wounds for 2 of 7 patients with wounds (#7 and 8).

Client record review and staff interview the agency failed to measure wounds for 1 of 7 patients with wounds (#9) Findings included: Review of a policy "Wound Assessment", revised 08/17 was received from Employee #2 on 11/14/19 at 2:12 p.m.

The agency failed notify the physician of a potential adverse drug reaction in 1 of 1 patient receiving IV (intravenous) antibiotics (#1).
